- In agreeing to the Terms of Business, you confirm that you are the legal owner / loaner of the animal referred for treatment.
- All animals treated by Kirsty Rawden Veterinary Physiotherapy that are under treatment from the vet require a signed and dated Veterinary Consent Form.
- Kirsty Rawden Veterinary Physiotherapy reserves the right to refuse treatment to any animal.
- If your animal has UNDIAGNOSED LAMENESS they must first see a vet.
- You must notify Kirsty ASAP to rearrange your appointment if your animal develops a lameness prior to your appointment. Treatment can commence once the vet is happy and gives written or verbal consent. (cancellation policy applies).
- Whilst every care is taken of the animal undergoing treatment, all animals receive treatment at their owner’s risk.
- All new appointments are required to be paid in full at the time of booking, appointments will not be secured until this is paid..
- All other payment for services are due on the day of the treatment. Payments must be made via bank transfer to the account name ‘Kirsty Rawden Veterinary Physiotherapy’ account number: 76188923 sort code: 04-03-33.
- Cancellation Policy - Any appointment cancelled or rearranged within 48 hours will be charged at 50% and within 24 hours 100%. No further appointments will be able to be made until payment is received.
- If your horse has an infectious/contagious condition or open wounds you are required to inform us with at least 24 hours notice and reschedule your appointment. Your horse must have been free from symptoms of sickness for 48 hours before commencing a treatment.
- Owners are required to notify Kirsty Rawden Veterinary Physiotherapy if, during a course of treatments, the horse’s condition worsens, or if the veterinary surgeon advises that the treatment is to be stopped or suspended.
- We kindly request that you have your animal as clean and dry as possible prior to treatment.
- Kirsty Rawden Veterinary Physiotherapy reserves the right to use video footage/photographs taken during sessions.
- Animals will usually require 48 hours off physical activity so please be mindful of this when booking an appointment.
Privacy Policy: The personal data that we collect about you will include data relating to your name, address and contact details, and the health/medical history of your animal. This data is collected as a way of identifying individual clients. All data will be stored securely and kept for up to 7 years (required for insurance purposes) and will be used by Kirsty Rawden Veterinary Physiotherapy for communication with you. The data collected will be used as a record of your animal’s treatment and any changes that may occur in their health. You have the right to request access to your data, request corrections (if any errors in the data are identified) or deletion at any time. Information will be shared ONLY with your animal’s vet or other veterinary professionals involved in their care for the purposes of keeping them informed of progress and changes with therapy.
